OBTAIN
A REPEAT PRESCRIPTION ?
If you are required
to take long-term medication, this may be available on repeat prescription. You
will be provided with a slip itemising your repeat medication. To request your
prescription, tick the box next to the item required and place in the prescription
request box provided. Please allow at least two clear working days (excluding
weekends) for your prescription to be ready for collection. You may post your
repeat prescription request with a stamped, addressed envelope (SAE) for its return.
Repeat prescription requests cannot be taken over the phone.

OBTAIN PRIVATE CERTIFICATES AND LETTERS ?
Certain types of medical certificates, medical examinations or information for
third parties requested by patients are not provided free under the NHS. You will
be charged a standard British Medical Association recommended fee for these services.
Details of fees are available at reception.
Sometimes employers ask their employees to obtain medical certificates from the
doctor during their first week of absence from work. You are allowed to complete
a self-certification for the first week of absence and there is no contractual
obligation placed on NHS doctors to certify such absence. Therefore, if such a
sick note is required, a charge will be made for the issue of a private sickness
certificate.
